People who have chronic illnesses or those that require constant medical attention but are not qualified to stay at hospitals for long periods of time, often obtain the services of home care coordinators to help them. Home care coordinators formulate care plans, ensure that these plans are implemented, and are responsible for the overall wellbeing and health of the patients they are assigned.

Most home care coordinators are required to possess licenses to practice as registered nurses if they want to work in this position. Since working in this position means that you will be standing for long periods of time, it is important to be physically dexterous.

Home care coordinators provide direct in-home care to their patients by performing a lot of coordination work. For instance, they may be expected to contact physicians and insurance companies to ensure that plans of care are properly implemented and that each service is paid for. In some instances, they may also find themselves in a position where they need to take important decisions such as admitting patients in hospitals.

As a home care coordinator, you will be required to possess critical thinking skills, decisive judgment, and be able to work with minimal supervision.

• Assess patients’ conditions to determine the severity of disease or condition
• Talk to patients and families to determine patient personalities and in-home support
• Work with discharge planning teams to refer patients needing other sources of care
• Coordinate efforts with physicians and nurses to establish coordinated care plans
• Contact insurance companies to obtain insurance approvals
• Act as a facilitator of in-home treatment team while giving and soliciting feedback on patient and family support progress
• Collaborate with correlating community and social services to ensure that appropriate resources are being utilized
• Develop and implement home care services for patients and ensure the efficacy of each program module
• Perform assessments of clinical, psychosocial, and demographic information to identify each patient’s problems
• Collaborate with healthcare professionals to ensure delivery of appropriate care to each patient
• Ensure that all home care workers are properly trained to handle emergency situations and regular care provision
• Interview patients in facilities to determine if they need in-home care
• Make recommendations for changes and/or alterations to existing plans of care
• Coordinate cased management duties including coordinating appropriate care, including community services
• Encourage family members to be actively involved in in-home care activities
