Dear Ms. Hernandez:

Please accept this letter as my formal notice of resignation from the position of Healthcare Assistant from Adelaide Health System on a 2-weeks notice, effective April 16.

At the beginning of July, I am expected to join Kansas State University for my bachelor’s program. I had always aspired to pursue further education in healthcare and could not pass up on this opportunity.

Working in the role of Healthcare Assistant has not only provided me with a platform to gain experience; it had also given me an opportunity to work with one of the most excellent healthcare providers in the state for which I am truly grateful.

The time that I have spent at Adelaide Health System and the experience that I have gained here will prove to be very useful for my bachelor’s degree. I am incredibly grateful for the patience that you have exercised in getting me trained. I have not only learned the various tricks of the trade but have also discovered my inherent compassion.

As my contract dictates, I am available to train my replacement and hand over my duties to him or her. I would appreciate it if you would let me know of any other responsibilities that you would like me to handle as part of the leaving process. It was an enjoyable experience working with Adelaide Healthcare System, and I anticipate a future position here.
