Dear Mr. Doe:

This letter is my formal resignation from St. Louis Nursing Home, effective two weeks from now. My last working day with your facility will be  January 10th, 2014.

This was not an easy choice to make on my part. The past three years with St. Louis Nursing Home have been very rewarding. I have taken extreme satisfaction in working for patients of different ages. I am proud of being a part of a very successful nursing team dedicated to a quality patient care delivered on time.

Thank you for giving me a chance for career development. Your guidance will be helpful for me throughout my professional life. I wish you and my colleagues all the best. I will remain available for handing/taking during this two week’s transition period.

Sincerely,
